A summary of information can be found here.
You must be an Illinois resident and have one of the qualifying conditions. A physician will then need to fill out the Medical Cannabis physician form and verify that you have one of the 40+ qualifying conditions. Note: this is not the same as prescription.
Follow the checklist on the back of the application to ensure you have the proofs of residency as well as age and identity included with your paperwork. Your application as well as your physician’s form will then have to be sent to the Illinois Department of Public Health. Note: You can not hold a school bus permit or CDL and not be an active duty law enforcement officer, correctional officer, correctional probation officer or firefighter.
After receiving your state issued medical card you can visit Nature’s Treatment and begin learning about what products are right for you and your symptoms. NTI staff will educate you on the different products available. You will then make your first purchase at the dispensary. Note: We are cash only at this time.
